Must be submitted for all SMCP inmates. Use this form to request bed space for an SMCP-eligible inmate.
Inmate Release form
Use this form to notify the NCSA that a SMCP inmate has completed their sentence and been released from custody.
Inmate Transfer Acceptance form
Use this form to notify the NCSA that a SMCP inmate transfer has been accepted by the receiving county.
Use this form to request a SMCP inmate’s transfer to NC D.A.C pursuant to G.S. 148-32.1(b3).
Inmate Transport Waiver form
Use this form if an inmate declines transportation back to county of conviction.
Out of Jail Medical Service form
Use this form to notify the NCSA that a SMCP inmate was transported
out of jail for medical services and to ensure reimbursement for mileage
and personnel hours.
Inmate Space Allocation form
For “receiving” counties only. Submit this form to update the SMCP Office of the maximum number of inmates the Program may send to your county.
